What is Social Prescribing?

Many things that affect your health cannot be treated by doctors or medicine alone. Like loneliness, debt, or stress due to financial pressures or poor housing. Social prescribing connects people with non medical support to help address these issues and other unmet needs. 

Our social prescribing service supports anyone aged over 18 years. 

We offer support with: 

  • Loneliness and Isolation
  • Social connections and Activities 
  • Physical health and emotional wellbeing 
  • Money, Benefits and Debt 
  • Family and home life 
  • Employment, Education and Volunteering 
  • Practical Support 
  • Carers support 

A social prescriber can help someone who is isolated join a befriending group, an art class or a community based project in line with the persons interests. Connect someone struggling with financial stress to a service that helps with managing debts or claiming benefits. 

Support someone in need of lifestyle changes such as weight management or quitting smoking.

What to Expect at Your Appointment

The social prescribing team offer both in person and telephone appointments.

At your appointment the social prescriber will take time to listen to you. We are led by you and our support focuses on what matters to you.

The social prescriber will support you to explore services and connections in your local community supporting you to gain choice and control over your health and wellbeing.
