Community Physiotherapists

There are a team of physiotherapists available for all our patients. Your doctor will refer you for treatment if he or she feels that you will benefit from physiotherapy.

Health Visitor are based at the Surgery and offer a number of health services and can be contacted on 01785 712841.

Health and Wellbeing Coach

The aim is to help people (18 years and over) gain and use knowledge, skills and confidence to take charge of your care.

They will help you identify your health and wellbeing goals and help you get there. We will:

  • Meet to discuss what you want to change
  • Create a plan and set goals together
  • Take it one step at a time

We're here to help if you are feeling overwhelmed, discouraged or like you don't have enough time.

Midwifery Team

Community midwives provide antenatal and postnatal care to Practice patients. A clinic is held at the surgery on a weekly basis. Your midwife will provide contact details and information about the clinic.

Social Prescriber

The aim is to connect people (18 years and over) to community groups and statutory services for practical and emotional support.

A social prescriber will work with you to identify your area of need, giving you time, focusing on ‘what matters to me’ and taking a holistic approach to health and wellbeing.

This covers a wide variety of areas, non-medical, as social prescribing helps different people in different ways. For example your concern could be related to

  • Social Care Needs
  • Housing
  • Finances
  • Isolation

A social prescriber will then help connect you to the right services or organisations. For example a local club or activity, or an organisation for mobility aids or carers.

Book an appointment via reception or self refer via Patient Access.

Wellbeing Worker

The aim is to empower people (18 years and over) with the tools to improve their wellbeing.

This may be regarding, but not limited to:

  • Stress
  • Anxiety
  • Bereavement
  • Low mood

This service provides low level intervention for patients in primary care.

We can work together to meet your wellbeing needs and future goals through self help resources, psycho education, normalising experiences and encouraging self reflection.
